Design a pilot evaluation package that defines the study framework, specifies quantitative and qualitative KPIs (including patient‑reported outcomes and experience measures), and produces a baseline vs. post‑intervention data collection and analysis plan. Include guidance for translating findings into clear summaries for funders and partners. Specifics will be provided.

This pilot will validate how iHealth services improve access, reduce patient burden, and produce potential healthcare cost avoidance for people with complex chronic conditions. Demonstrated results will strengthen partnership and funding opportunities and help scale a platform that connects patients to high‑quality care and resources.

We will work collaboratively with volunteers, clinical, product, and community stakeholders to refine measures and methods. Volunteers will be given organizational context and access to available data sources; details and materials will be provided during interviews to ensure smooth handoff and regular check‑ins.
